The Road to Purity Survey is completely anonymous. No personal or identifiable information is collected during the survey itself. Upon completing the survey, participants will be directed to a separate form where they can voluntarily provide their information (name, email address, and mailing address) to enter the giveaway.The personal information provided will only be used for the following purposes:
-To notify the winner of the $100 Visa Gift Card.To mail a free book (if applicable).
